While reading the increasingly interesting AOK discussions about social networks, inspired by the recent Star series with Patti, I came across Judith Meskill's blog. She talks about something I have been contemplating about: the results of the Shuttle disaster and the role of the organizational learning and knowledge transfer:

In most articles you can read the the knowledge transfer in the safety practice was actually insufficient - due to organizational, management and political reasons. However, it was the same Safety board that initiated a couple of years ago the communities of practice program by Steve Newman (called the PBMA). After reading some more on PBMA it seems to me that these communities were left to be very voluntary:
